If this is an emergency situation, please contact Campus Safety at 816-501-4010 or call 911 immediately.

Incident Reporting Form
This form is for reporting violations of the Student Handbook. Examples include drugs, alcohol (including amnesty), harassment, disorderly conduct, quiet hours, visitation, etc. If you have questions whether a violation falls under this category then please contact the Assistant Director of Residence Life at 816-501-4126.

COVID Positive Reporting Form
The University is requesting that any member of the campus community who has tested positive for COVID should self-quarantine following CDC guidance and submit this form immediately upon testing positive.

Sexual Harassment Reporting Form
This form should be used to report any incident involving Rockhurst's Sexual Harassment Prevention and Response Policy (including sexual harassment, sexual violence, domestic violence, dating violence, stalking, or any other form of sex discrimination).

Please note that all staff, faculty, and administrators (excluding those working in a confidential capacity such as counselors, medical providers, and clergy or campus ministry staff when providing pastoral care) are required to disclose any reports of sex discrimination and may not make anonymous reports.

Rockhurst University encourages all victims to make a report with law enforcement agencies and seek immediate medical attention at the local hospital, especially in instances of sexual violence.

Concerning Behavior Reporting Form
This form is for sharing concerns you have regarding the well-being of others on campus or concerning behavior that may not be considered a conduct violation but is worrisome nonetheless.

By sharing your concerns, you are helping to maintain Rockhurst as a healthy and safe campus environment. Once received, the information will be reviewed and you may be contacted for additional information. If you wish to make an anonymous report of concerning or worrisome behavior, the report will be evaluated and a determination will be made for what follow up (if any) is necessary to address the concerns. You are strongly encouraged to provide your name and contact information to allow us to call or email if we have additional questions regarding your report. See the Rockhurst University Disruptive and Dangerous Conduct Policy.

Discrimination, Harassment and Retaliation Reporting Form
This form is for reports of discrimination, harassment and retaliation on the basis of race, color, creed, religion, sexual orientation, marital status, national origin or ancestry, age, gender, disability, citizenship or intended citizenship status, veteran status or any other status protected by law (“protected status”). See the Rockhurst University Policy Against Harassment, Discrimination and Retaliation. (Discrimination, harassment and retaliation on the basis of sex are governed exclusively by the University’s Sexual Misconduct Prevention and Response Policy)

Neighbor Reporting Form
Please complete the following report to the best of your ability in order to provide Rockhurst University as much information as possible to investigate. This report does not take the place of contacting Rockhurst University Security at 816-501-4010 if the incident is in progress. Reporter is allowed to remain anonymous.
